I tried to be as detailed as possible, as I get irritated by tutorials that tend to gloss over important details...but it did get a little long. I've separated out the part about hair, I'm making a different tutorial focusing on that.

Edit: I added a 25 pixel margin to the left and right sides. I was bothered by the lack of psychological stretching room on the edges....in other words, the image was cropped too close to the text.
